A form like any other

This is an ordinary quote form. Use an address you can actually receive mail at — you'll need it in step 4, and it's how the record gets tied to you rather than to nobody.

step 02 · proof

What exists now that didn't before

The moment you submitted, the exact disclosure you read, the recipients named in it, the purpose, and the timestamp were hashed into one fingerprint. Change any of it afterwards and the fingerprint stops matching.

step 03 · verify

Don't take our word for it

This is what a carrier, a regulator, or a buyer sees — fetched live from the public verification endpoint. No account, no API key, no relationship with us required.

step 04 · withdraw

Now change your mind

Consent you can't withdraw isn't consent. Prove the address is yours and the certificate moves to revoked — a ten-business-day deadline starts for the business to stop contacting you, and nothing is deleted: the record stands as evidence of both the agreement and its end.
